Care Nexus (“Care Nexus,” “we,” “us,” or “our”) operates the website located at www.carenexus.com and any other related websites or mobile applications with links to this Privacy Policy (collectively, the “Site”). Through the Site, we operate an online technology platform enabling members (“Members”) to receive healthcare and/or telehealth services rendered by various licensed healthcare providers (the “Services”). We developed this Privacy Policy to explain how we collect, use, and disclose information from and/or about you when you use the Site.
IMPORTANT: THE SITE WILL BE COLLECTING AND TRANSMITTING PERSONAL, MEDICAL, AND HEALTH-RELATED INFORMATION ABOUT YOU. BY USING THE SITE, YOU AGREE THAT WE CAN COLLECT AND USE YOUR PERSONAL AND OTHER INFORMATION AS DESCRIBED IN THIS PRIVACY POLICY. IF YOU DO NOT AGREE, PLEASE DO NOT USE THE SITE.
• Business Associate Notice: In certain cases, Care Nexus may act on behalf of healthcare provider clients (the “Care Nexus Customers”), and your Protected Health Information (PHI) may be governed by a Business Associate Agreement between Care Nexus and that provider. If you are a patient or other end user of one of our healthcare provider clients and have questions concerning the processing of your personal information, please direct those questions to the relevant provider.

• “Personal Information”: Information about you that is personally identifiable, including but not limited to contact information (name, address, email, telephone number), personally identifiable health or medical information (“Health Information”), and any other non-public data associated with such information.

We do not knowingly allow individuals under 18 to create user accounts for the Site, and we do not knowingly collect Personal Information submitted by individuals under 18. Only a parent or legal guardian may create a user account for a minor and provide any necessary Personal Information about that minor. If we discover that Personal Information about a minor has been submitted by someone other than a parent or legal guardian, we will remove such information from our systems as soon as possible.
